Lesson 11 form seems to stack labels and fields

I'm having a problem getting the form to render correctly. The form seems to stack the label and the formfield on top of each other for every row.

Facing the same issue due to a holder div being also assigned the form-control class. Currently as a hot-fix just implemented the form-control class using JS...
