The authentication plugin should be able to report an error state (in addition to pending / authenticated / not-authenticated) to signal that the user is not authenticated due to a problem error.

This gives the UI the opportunity to report the problem.
The current behaviour can trigger redirect loops (e.g. trails -> keycloak -> setup problem -> trails ..).
