Easy error report

It should be very easy for users to submit an error message. A form that asks for the relevant information and submits the data to github issues would be a good solution.

This is a bigger topic. As a first solution, I would just provide contact information and some questions to get qualitative reports. The form and ticket system connection could be added later.
